| 1. Collect sales partner form from any Branch of Equity Assurance Plc or download it from the website. |
| 2. Go to Equity Emergency Express card website. [www.equityassuranceplc.com] |
| 3. Complete and submit the Sales Partner form. |
| 4. You will then receive a Partner code with the following documents. |
- Memorandum of understanding
- Sales manual
- Partner code
- Sales Sticker
- Sales promotional Items

| 5. You will then open an account with a UBA Branch. |
| 6. You will submit the account number, so that it can be tied to your code. |
| 7. You will ensure that you put your Partner Code on the Bank slip when making a payment for any sales made. |
| 8. Your commission will be paid directly into your account on the first working day of every month. |
